Responsibilities: – Adhere to and maintains required levels of performance in both Coding accuracy and productivity – Identify appropriate assignment of CPT and ICD-9 Codes for Physician and facility services provided in an Observation service setting, and Inpatient setting – Understanding of ICD-9 Coding in relation to DRGs – Abstract additional data elements during the Chart Review process when coding, as needed – Maintain a thorough understanding of assigned Client Coding specifics – Perform Coding duties as appropriate according to pre-determined schedules – Review and maintain a record of charts coded, held, and/or missing – Provide documentation feedback to Providers, as needed, and queries physicians when appropriate – Maintain up-to-date Coding knowledge by reviewing materials disseminated/recommended by the QM Manager, Coding Operations Managers, and Director of Coding/Quality Management, among others – Participate in Coding department meetings and educational events – Experienced Inpatient Coders Hospital – Reads through the hospital/medical records and accurately assigns and sequences diagnoses and procedure codes in accordance with coding books, Medicare policy, CCI Edits and client coding guidelines; – Maintains no less than 95% coding accuracy for all specialties and no less than the CPH target (chart per hour) on his/her designated specialty; – Consistently demonstrates time awareness: strives to meet deadlines; reduces non-essential interruptions to an absolute minimum; – Meets departmental productivity standards for coding and entering inpatient and/or outpatient records; – Participates in coding meetings and education conferences to maintain coding skills and accuracy; – Demonstrates willingness and flexibility in working additional hours or changing hours; – Demonstrates thorough understanding on how position impacts the department and hospital; – Demonstrates a good rapport and works to establish cooperative working relationships with all staff offshore and onshore. – 100% non-voice account – Flexibility on projects, specialty designation, coding software, roles&responsibilities, etc. – Sitting, typing and looking at computer screen for 7.5 hours – Carrying of, and reading through, different coding books – Navigating through multiple systems/softwares/web pages – Determine whether the coding assigned by the client was properly assigned based upon review of the medical documentation and application of the coding guidelines – Develop and apply appeal arguments to defend the coding decisions of our clients while being able to address and refute the coding determination made by the CMS contractor – Draft appeal letters, including the coding argument, to support our client’s coding decision – Provide feedback to management regarding trends or patterns noticed in client coding for discussion with client – Participate in ALJ hearings as required to defend client coding -Perform other duties as may be assigned by management Qualifications Qualifications: – Coding credential required (RHIA, RHIT, CCS, CCS-P, CPC, CPC-H); will also consider RN or LPN with coding experience. – RN and LPN will be considered as long as the minimum requirement of two years facility inpatient coding experience requirement is met.
